If you have already scheduled an appointment with one of our providers, you can save time during your first visit with us by registering online. To register online follow the directions below:

  1. Click on the REGISTER ONLINE link on the right.
  2. On following page complete the PRIMARY CLIENT INFORMATION making sure to fill in all required sections (shaded in yellow).
  3. If you will be using Health Insurance complete the PRIMARY INSURED and PRIMARY INSURANCE INFORMATION sections.
  4. After you have completed the initial page click SAVE.
  5. On the next page please print out and complete the Patient Questionaire and any other forms requested by your LHS provider.
